2026 Weaver PTA Honorary Service Award Nomination Form

The volunteers at Weaver Elementary School play a vital role in our school’s success. Each year, the Weaver PTA honors dedicated volunteers for their service to our children through the Honorary Service Award (HSA) Program.

We are looking for those special individuals who have gone above and beyond in their service to our students, teachers, and school community. Too often, these extraordinary people go unrecognized because they work behind the scenes. Too often, they are not nominated because we assume someone else will do it. Be that someone! Please take a few moments to let us know who you believe deserves to be recognized.

Things to Consider

  • Breadth of service to our community – Consider the range of service provided. How varied is this person’s involvement with students, staff, and parents? How many different roles or “hats” does this person take on at Weaver?
  • Depth of service to our community – Consider how knowledgeable this person is in the areas they serve. Do they take on leadership roles? Do they build on prior years’ experience to improve and expand their contributions?
  • Length of service – Consider how long and how consistently this person has contributed to our community in meaningful ways.
  • Impact of service – Consider how our school community has benefited from this person’s efforts.

All nominations will be reviewed, and recipients will be selected by the HSA Selection Committee, composed of PTA members. Awardees will be honored at a special flag ceremony, and a donation will be made on their behalf to the California State PTA’s scholarship and grant programs.

You may nominate more than one person by submitting a separate form for each individual.

All nominations are due by Friday, April 3rd, 2026

 

 

Below is a brief description of each award:

  • Very Special Person Award (VSP) – This award is given to recognize an individual or organization for their service to PTA.
  • Honorary Service Award (HSA) – This award is given to recognize an individual or organization for outstanding service to children and youth in your community, going above and beyond what is asked of them.
  • Continuing Service Award (CSA) – This award is given to recognize an individual or organization for ongoing or long-time service to children and youth, providing support year after year.
  • Outstanding Teacher Award (OTA) – This award may be given to recognize a teacher for outstanding service to children and youth through PTA, school, or community.
  • Outstanding Administrator Award (OAA) – This award may be given to recognize an administrator for outstanding service to children and youth through PTA, school, or the community.
  • Elected Official Honorary Service Award (EOHSA) – This award may be given to recognize an elected official for outstanding policy work and/or legislation that positively impacts the lives of children. This award may be given at the local, state or federal level. This award is not to be used as an endorsement of any elected official, but to celebrate the work of the elected official.
  • Golden Oak Service Award (GOSA) – This award is given to recognize, as the most prestigious of the Honorary Service Awards, an individual or organization that has made significant contributions to the welfare of children and youth in a school or community.
