Below is a link to the California PTA required insurance forms if you will be providing ANY service for ANY PTA related event whether it is on campus or at another location.

All Vendors must submit this paperwork to their Main contact along with a signed HOLD HARMLESS agreement, prior to the scheduled event.



Weaver PTA Chairs & Committees: 

Below is a link to the PTA reimbursement form. 

Use this to be reimbursed for expenses for all PTA related events.

The expenses must have been approved by the board and or the PTA president, VP or VP of Ways and Means.

Once completed, email the form and all receipts (scanned) to Amanda Baltz,  Please drop off the physical copy of the form and the physical receipts in the PTA  basket in the school office.
